RULES
1. CONTEST PERIOD
The 705 Realty & STAY WELL HEALTH Total Health Review Giveaway (the "Contest") begins on [start date] at 12:00 AM ET and ends on [end date] at 11:59 PM ET (the "Contest Period"). Entries submitted outside the Contest Period will not be considered.
2. ELIGIBILITY
The Contest is open to legal residents of Ontario, Canada, who are 18 years of age or older at the time of entry.
Employees, representatives, and agents of Keller Williams Experience Realty, Brokerage o/a 705 Realty (the "Sponsor"), STAY WELL HEALTH, their affiliates, advertising and promotion agencies, and members of their immediate families (spouse, parent, sibling, or child) or household members are not eligible to participate.
3. HOW TO ENTER
No purchase is necessary to enter or win. To enter, participants must complete all of the following requirements during the Contest Period:

Follow Stay Well NP (@staywellnp) on Instagram.
Like the official Total Health Review Giveaway video posted by @705realtygroup on Instagram.
Complete the official Contest entry form by providing their full name, email address, and phone number.
Bonus entries: Participants may receive one (1) bonus entry for each eligible person they tag in the comments on the official giveaway video, subject to verification by the Sponsor.
All standard entry requirements must be completed for an entry to qualify. Automated entries, duplicate entries, or entries submitted through fraudulent means will be disqualified. The Sponsor reserves the right to refuse any entry that appears ineligible, suspicious, or does not meet the Contest requirements.
4. PRIZE DETAILS
One (1) winner will receive:
One comprehensive Total Health Review provided by Nikki Mayes, Nurse Practitioner at STAY WELL NP (Nurse Practitioner Led Health Care), valued at $450 CAD.
The total approximate retail value of the prize is $450 CAD.
The prize must be accepted as awarded, is non-transferable, and cannot be redeemed for cash. The prize is subject to the provider's eligibility requirements, service terms, appointment availability, and any booking or redemption deadlines communicated to the winner. Further details regarding the service and any applicable restrictions will be provided to the winner.
The Sponsor reserves the right to substitute the prize with one of equal or greater value if the advertised prize becomes unavailable, subject to applicable law.
5. WINNER SELECTION AND NOTIFICATION
One (1) winner will be selected by random draw on or about [draw date] from all eligible entries received during the Contest Period. The draw will be conducted by the Sponsor or its designated representative.
The selected winner will be contacted via email and/or phone using the information provided on their entry form. If the selected winner does not respond within [response period], fails to meet the eligibility requirements, or does not correctly answer the skill-testing question, an alternate winner may be selected.
6. SKILL-TESTING QUESTION REQUIREMENT
To claim the prize, the selected winner must correctly answer a time-limited mathematical skill-testing question without assistance.
7. GENERAL CONDITIONS
By entering, participants agree to abide by these Official Contest Rules and the decisions of the Sponsor, which are final and binding, subject to applicable law.
The Sponsor reserves the right to modify, suspend, or terminate the Contest in the event of technical issues, fraud, or other circumstances beyond its reasonable control, subject to applicable law.
The Sponsor is not responsible for lost, late, misdirected, incomplete, or invalid entries due to technical, internet, or other issues beyond its reasonable control.
The prize is a health service provided by STAY WELL HEALTH. Contest entry does not establish a nurse practitioner-patient relationship, guarantee eligibility for the service, or guarantee any particular health outcome.
8. PRIVACY AND DATA COLLECTION
By entering the Contest, participants consent to the collection and use of their personal information by the Sponsor for the purpose of administering the Contest, verifying eligibility, and contacting the winner.
Personal information will be handled in accordance with the Sponsor's applicable privacy practices. Entry information will not be shared with STAY WELL HEALTH for unrelated marketing purposes without the participant's consent. Any separate marketing communications will require appropriate consent and will include an option to unsubscribe where applicable.
9. RELEASE AND LIMITATION OF LIABILITY
By participating in the Contest, entrants agree to release and hold harmless the Sponsor, STAY WELL HEALTH, and their respective affiliates, employees, and representatives from liability arising from participation in the Contest or acceptance or use of the prize, to the extent permitted by applicable law.
10. GOVERNING LAW
The Contest is subject to all applicable federal, provincial, and municipal laws and regulations and is void where prohibited by law. Any disputes arising from this Contest will be governed by the laws of the Province of Ontario.
11. WINNER ANNOUNCEMENT
Following winner verification, the Sponsor may announce the winner on its website or social media pages, subject to applicable privacy requirements and the winner's consent where required.
12. INSTAGRAM DISCLAIMER
This Contest is in no way sponsored, endorsed, administered by, or associated with Instagram. By entering, participants release Instagram from any responsibility related to this Contest.
